**PLEASE READ ALL GRANT MATERIALS BEFORE FILLING OUT THE APPLICATION**

Granting Miracles Board of Directors will select applicants who are uninsured  for fertility treatment for a grant that will be used toward fertility treatment at any fertility clinic within the U.S.

Completed applications must be mailed with a non-refundable application fee of $100 made payable to Granting Miracles via check or money order. Applications that are late or incomplete will not be reviewed.

The application due date:  March 15^th, 2024

We would love to offer grants to every applicant but, unfortunately, not all applicants will receive grants. Grant amounts vary based on needs, up to $15,000. Grants also include discounted prices for services at Georgia Reproductive Specialists / Shady Grove Fertility. We also offer a grant “matching” program. Grant applicants may request that Granting Miracles “match” the funds that they have already saved/raised for their upcoming fertility treatment. Once the grant amount is determined – funds are paid directly to the patients fertility clinic for the cost of treatment.

There is a $100 application fee for each application submitted. This fee is NONREFUNDABLE. If you decide to withdraw your application between the time that you submit and the board has made a decision, Granting Miracles will not be able to refund the fee. The application fee is considered a donation and the foundation can provide any paperwork you would need to document the donation. The fee must be submitted with your complete application made payable to Granting Miracles via check or money order.

The grant process once an application has been submitted:

• The application is received by Granting Miracles on or before the due date.
• The application packet is received and reviewed to ensure that it is complete.
• All applicants will receive an email within 7 days that their application has been received.
• Application is reviewed and confirmed that there is a financial need (the applicants insurance does not cover IVF).
• Application is reviewed by our Board of Directors.
• Applications are presented to the entire Board of Directors and final applicants are selected.
• Background check is completed on final applicants.
• Final decision regarding grant recipient is made.
• Couple is notified that they are receiving grants.
• Once the grant recipient couple has accepted, we then notify all other applicants of board decision.

Please do not contact the foundation during the review process.

The process does take approximately 6 weeks. Granting Miracles is an all volunteer board. We understand that this is a trying time for infertility patients, but know that we are working diligently to select a grant recipient within a reasonable time frame. We will contact you if we need any additional information. We do not take these decisions lightly so we will not rush them.

Requirements for the Grant:

• Applicants must demonstrate financial need and be uninsured for fertility treatments.
• Grant recipients must complete all IVF (fresh and frozen cycles) within 365 days of the award date.
• All monies will be paid directly to the patients fertility clinic; no monies will be given directly to the patient.
• If you are applying for a grant “match” you will need to specify in your application the amount that you are requesting, the fertility package that you are trying to fund, and submit bank statements to substantiate that you have raised/saved the amount that you are requesting for the match.
• The grant may not be used to reimburse the patient, nor the clinic, for the services already received; the grant cannot be used for services already received; the grant cannot be used for services that have already been performed, nor loans that have already been secured; the grant is used towards future cycles.
• All grant recipients will be subject to a criminal background check and a credit check.
• You must meet the American Society for Reproductive Medicine definition of Infertility (i.e. blocked tubes, unexplained infertility, endometriosis, PCOS, male factor, female factor, etc.). A fertility specialist must officially diagnose infertility.

Reminders about the Application:

• Applications must be submitted on or before the due date.
• Late applications will not be considered.
• Application fee of $100 needs to be submitted at the same time of the application.
• PLEASE include the completed checklist with the complete application.
• Make a copy of the entire completed application packet and retain the copy for your records.

The FULL APPLICATION PACKAGE must be printed, completed, signed, and mailed.
